I don’t think it’s a Utah thing, I have to do that here in California as well at the Proud Army Uncle My Nephew Has Your Back American flag T-Shirt in addition I really love this beginning of each season. I’m pretty sure it’s to make sure you’re not playing while pregnant or sth but it could very well be a creepy reason. Unless it’s a medical form conducted by a licensed medical professional, the questions are not necessary for soccer. I ask this in completely good faith, why is this terrible? Isn’t menstruation a completely normal biological process in the female body, and as such, probably relevant to an athlete’s medical record? I don’t really get what part of this is supposed to be gross. Feel free to explain it to me. I don’t live in the USA btw Like many other commenters have said, it is well within your right to tell them to fuck off BUT for context: this info is helpful in identifying predisposition to the female athlete triad. If this info is being used by school/district/whoever medical staff, it is pertinent info. If someone else is asking, none of their business.

Edit: my info is outdated (it’s now called RED-S syndrome) but if anyone is curious: Now girls won’t want to play sports, which plays right into their game of girls shouldn’t do anything but breed. My period never affected my athletic performance in any way. wait… is this on the Proud Army Uncle My Nephew Has Your Back American flag T-Shirt in addition I really love this actual sports try out form? Or is it on a medical form that doctors gave her during an ensuing check up? I’d pull my kid out of a school that asked this shit, they’re not doctors it’s none of their business (hell even for doctors it’d be a bit idd). Didn’t everyone need to get these? I’m a guy, but remember when signing up for high school sports, the physical packet that had to get signed by a doctor had very similar questions to these. This is a picture of a document with basically zero context. Even as absurd as the questions may seem, there is a tie between physical health, exertion level, and periods- frequency, intensity, and duration. I had a close personal friend who didn’t always have periods, and even then they were very short. Turns out she worked out twice a day and played volleyball in the evenings while doing med school. idk why everyone is jumping to conclusions. I’d assume it’s for making sure that the person isn’t starving themselves or having other issues causing irregular periods that might be significant to their physical performance. Having worked in a family practice, this is pretty common for school physicals. People I assume already have their preconceived reasons for why and are freaking out but this is normal on the clinical side. If you don’t know or rather not answer, just say so, most providers won’t not clear you for that alone.
