I don’t think a company will be waiting more than that to hear back from a candidate when they have someone already waiting to accept the offer. Send an email directly to both the head of HR and whomever the supervisor reports to (VP level or higher). Express how disappointed you are in their handling of their error, and how poorly it reflects on them as a company that they can’t take ownership of their mistakes. Say that while you were excited to accept the offer in the time communicated, their response to the issue has led you to seek employment elsewhere.
