It sounds like the DOT had issues in addition to his brakes with the vehicle. The guy took unnecessary shortcuts, thinking Mavis would slap a band-aid on the problem and everything would be fine. Turns out the band-aid was faulty and he didn’t wait for final DOT approval anyway.

Like I said I think Mavis got lucky with these set of facts.
