Her OB should have said something when she went for her first visit as a pregnant person. Her doctors should have known there was a problem the first time she tried to refill this medicine after becoming pregnant or the first time she saw the birthing OB. Provided OP listed her medications correctly when she filled out her new patient forms at least. Zoloft and pregnancy should have sent up a flag to the OB and GP. If they did not have software capable of tracking conditions, medicine and their side effects based on those conditions the doctor is the last line of defense here. They should have seen it in her chart and said something. The OB, GP, and hospital staff all failed this woman. This type of thing speaks to institutional failure. It should have been caught well ahead of time.
