I’m gonna play Devil’s advocate, just because I have the time. I have to state though that I live in Europe, and love my universal healthcare. I make the point that universal healthcare is fundamentally incompatible with the american mindset. At it’s foundation, universal healthcare relies on preventive care above all to remain affordable. And I don’t mean just doctor visits. We are talking about regulating what you eat, what you drink, what you breathe, your lifestyle. We are ok with this in Europe, we understand that regulations exist for our own benefit. Does that mean I don’t have the freedom to eat whatever I want for cheap? Sure it does, and I couldn’t care less. The safety net that protects me from losing my home, my job, a family member, is more important than McDonalds and Wendy’s. If America can’t even pass meaningful regulations on gun ownership, because amendments, freedom of whatever, forget about regulating the crap the food industry uses as ingredients to lower the price and increase demand.
I don’t see it, I believe it is impossible without addressing other issues first, and I don’t think those issues will be addressed anytime soon. Republican Americans are incredibly anti-tax and they think socialism is a bad word. Every time I’ve driven from Ontario to Florida (or another southern destination), on either the eastern seaboard route or through the Midwest route, it is shocking to see the difference from Blue states to Red states (and even between Red states) once you’re off the interstate. For example, on the way to Myrtle Beach, SC, the last leg of the drive is on state highways and county roads. When you cross from NC to SC the roads are immediately not cared for and the school busses look like prison busses from the 60s. I understand that differences in population density and the overall wealth/poverty of the tax base plays an important role in this, but so does the unwillingness to pay an appropriate tax to take care of these things in the first place.
I will never understand how they prefer to pay a for-profit insurance company for services from a for-profit hospital instead of paying less money in taxes for non-profit government run insurance for services from non-profit hospitals where everyone wins. If you’re making 500k a year you’re all the sudden paying 50k for health insurance whereas before you were paying 10-15 at most.
And to be clear it’s not super rare to make 500k. You can own a restaurant or be a plumbing contractor. So to a well off American with health insurance, they’d be paying more to gain less. The healthcare is free “at the point of use” but isn’t really free. The fact is it costs money. Either you can’t pay, or the government can’t pay. Look at Canada or the UK. Both healthcare systems on the verge of collapse, years long waiting lists for basic procedures. Might work well for a rich country like Sweden selling all that iron ore to Nazi Germany, relying on the US military for protection. But for countries with real spending priorities and obligations, some just prefer to let the individual save and prepare rather than being totally reliant on the state.

Another line of thought could be that most of your health conditions is based on your day to day choices. If someone puts more effort in being healthy, why should they be punished with a high tax cost from the overweight population. Just giving an example. The issue is that the cost of healthcare in the US is outrageous, and free healthcare does not necessarily reduce the the cost. I don’t necessarily know what it takes; either more doctors, more nurses, more healthcare facilities, more transparent pricing of procedures, an detailed end of life treatment, taxing High Corn Fructose syrup, government production of generic drugs and other essential medicine, or etc.
In my state of California of 39 million. That’s left leaning and generally economically well performing, there’s 16 medical schools which trains doctors. Sweden has 10 million people. Sweden has 7 medical schools per wdoms. America is a country that, more than any other, is set up that it is up to you to do your best to progress and to look after yourself. When you have been indoctrinated in to that mindset then accepting free healthcare as a good idea is admitting that you are worse than average. In theory I support socialized healthcare, in practice I think in America it would turn into a poorly run money making boondoggle for medical providers. I expect costs would go up, quality of service would go down. The current system is criminal, I think given the chance politicians and the insurance company would come up with something far worse. I fear the crazy ideologues on either side being in charge of determining access to care (we could completely lose abortion care, access to birth control, end of life counseling, etc). The Centers for Medicare and Medicaid Services (CMS) have problems that would be magnified if they suddenly controlled all of US healthcare. A lot of Americans fear government-run healthcare (free healthcare is a misnomer) because they don’t trust the government but for some reason trust large corporations. Some want it only because they trust the government more than corporations.

Part of the bind that we’re in is that the system is half socialism already, which creates huge disparity between the non productive who get totally free medical care at any time, any place, without ever having to pay a bill and the working class, who pay $12,000 a year in premiums, have a $6,000 deductible (really amounts to a catastrophic care plan) and can’t access health care. The working class pay for all of this through taxes, and those funds are shoveled over to the irresponsible (ie: illegal aliens, single parents who keep having kids, the addicted, the poor, etc.) The way it’s set up now, I have more incentive NOT to work because at least I can go to the doctor. Second point: the government creates, produces, does nothing. Anything the government touches gets ruined. There is no accountability. Bureaucrats become less productive paper pushers over time because there is no incentive to improve. And then you have a place like the DMV, where you have to sit in line for hours to get a piece of paper stamped to go wait in a second line. Rude, disprespecful public employees. Filthy conditions. Have you ever had the misfortune of having to communicate with IRS? Good luck. You’ll be on hold (phone) for hours or waiting six months for a letter to arrive. The government is the most inefficient organization there is. If the hospital bills private insurance $83.00 for a syringe, you can bet the government will be paying $600 for a syringe and there will never be enough tax revenue to cover the inefficient waste. IMO the whole system needs to be scrapped and started all over again from the framework, starting with transparent pricing, cash options and legitimate, accountable insurance that is not tied to government. In other words, capitalism. I think we now have 60 years of proof that socializing medicine is killing it.

We have some systems of it in place, but it’s rough. Personally I love the idea of free healthcare, but with the climate of our insurance and medical field, it’s just not going to happen well. Do you think the mayor of New York City would bother listening to crime advice from the mayor of Torrington, Connecticut? People like to treat all nations as equal but we aren’t. We’re not the same and what may work in some countries won’t work in others. Then there’s the practical answer; we have had a private system for decades. There is an entire industry and infrastructure that would need to be radically changed. That means hospitals, general practice doctors, specialists and so on would have to radically reorganize.

Treating goods and services as a right destroys the incentives to provide more of it at a lower cost and to improve those goods and services. Once health care is “Free”, spending on healthcare becomes political and no one likes to pay more in taxes so those costs must be contained, and will be by rationing, reducing the amount available, or by reducing pay to providers, which ends up reducing the number of providers, and thus reducing the amount of healthcare available.
