Which doors are open is done automatically, a driver probably could open the wrong doors on purpose but they would have to break through a load of safety interlocks to do so (ie it would have to be extremely deliberate). This is almost certainly a hardware aor software issue. Almost certainly TfL faults, unless it’s an issue with a recent software update to train control or signalling software. All train operators take this kind of fault very seriously, because of the obvious danger. Not something they can just shrug off.

This activates an alarm for the driver and puts the emergency brakes on. The driver has to apply a procedure/drive the train in a special mode to carry on to the next station, safest place to carry out the next step… For the next many stations the platform will be on the left. This is no good for the passenger with the stuck bag/coat etc if they want to get off. So the driver goes back, asks passengers to move away from the door in question, uses an emergency door open for just that door (which makes sense for the photo) and then unfortunately the only way to close them again is to reach outside and shut them manually. The train can then continue.
