I just have a gut feeling that the same people who would play this type of greedy malicious game in the first place might be the same people who might not inform you and bank on you not knowing any better.

One “trick” is to ask the insurance for the name and license number of the doctor in the insurance company that denied the claim / pre-authorization. Then, the claim will go to a real doctor who will likely approve it. If they don’t give it to you in 24 hours, report them to your state’s DOI. This is the site for MA residents.
